You cannot pay for an outcome you cannot see. Value-based healthcare and value-based procurement both rest on outcome data that has to cross an organisational boundary — from the facility that delivered the care to the payer, regulator or supplier being held to it. That crossing is exactly what a health information exchange is for.
Rewarding outcomes rather than volume means measuring an outcome across a whole episode, and an episode usually spans more than one provider. The exchange is what makes it readable end to end instead of as four disconnected encounters.
Contracting a supplier against reduced readmissions or faster recovery only works if those measures come out of the clinical record and reach the counterparty. Without an exchange, an outcome-based contract has no agreed source of truth to settle against.
Outcome data is still patient data. Sharing it for payment, procurement or population reporting runs through the same Consent Management Service, role-based access and audit trail as any other exchange — so a new commercial model does not need a new privacy model.
